Dependable Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Hospice Continuous Care team serving patients throughout the Tucson area.

In this meaningful overnight role, you will provide extended bedside nursing care and symptom management support for hospice patients requiring continuous care. You’ll play an essential role in ensuring comfort, dignity, and peace for patients and families during critical moments.

Your Responsibilities

As a Hospice Continuous Care LPN, you will provide skilled nursing support and compassionate bedside care for patients experiencing acute symptom management needs.

Daily Responsibilities Include:

• Deliver direct patient care in accordance with hospice care plans

• Monitor patient condition and promptly report changes to the RN case manager and hospice team

• Administer medications and treatments as directed

• Provide comfort-focused care and emotional support to patients and families

• Assist with pain and symptom management interventions

• Document patient care accurately and thoroughly

• Collaborate with interdisciplinary team members to ensure coordinated patient care

• Support patients and families with dignity, compassion, and professionalism throughout the overnight shift

Qualifications

• Current Arizona LPN license in good standing

• Current CPR certification

• Hospice, home health, long-term care, or skilled nursing experience preferred

• Compassionate and patient-focused approach to care

• Strong communication and organizational skills

• Valid Arizona driver’s license and reliable transportation

• Ability to work independently in a home setting

Education Requirements

• Graduate of an accredited Practical Nursing program

• High school diploma or GED required
