Aviva Family and Children’s Services Mission StatementCreating safe, supportive spaces, where children, youth and families can heal, build resilience, and experience belonging.Job SummaryUnder the supervision of the Program Lead or Program Manager, the Intensive Care Facilitator is responsible for the coordination, scheduling and facilitation of Child and Family Team (CFT) meetings. The Intensive Care Facilitator will conduct regular CFT meetings for each client on their caseload within the timeframe identified by the program and/or needs of the client/family. They will complete associated documentation including the Safety Plan, Outcome Measures, CFT Matrix and progress note. This position will collaborate with inter and intra-agency providers to coordinate care, monitor and ensure resources and linkages are accessible and appropriate for the youth and family. They are well versed in and are able to communicate to others the program philosophy, principles, and process. Services are provided primarily in the field including at clients’ residence, school, other community location, or in the office. The Intensive Care Facilitator is responsible for completing all required documentation in accordance with Agency, County, and COA standards.Essential FunctionsCoordinate CFT Meetings to develop, guide, and plan service delivery to meet the needs of the client and family in the home, community, or office based off family’ preference.Utilizes the Integrated Core Practice Model (ICPM) principles and components to engage with participants, identify strengths and underlying needs, and strengthen teaming across systemsCoordinate comprehensive care including facilitating cross-system communication, planning and linking participants to resources and other child-serving systemsCollaboration with community partners, formal and informal supports including, but not limited to, DMH, DCFS, Department of Probation, Regional Center, DPSS, etc.Ensure plans from all involved child services systems are integrated and identified goals and objectives are supported to support successful changeEnsures the CFT includes the client, family members, clinical team members, County representatives, and natural supports identified by the client and familyWorks to ensure active participation in the CFT process by all membersWorks with family and team to identify strategies for achieving treatment goalsDevelops CFT meeting agenda reflective of the voice of the client and family, Matrix, progress note and other supporting documentationResponsible for scheduling and managing the logistics of the CFT Meeting Process including location, time, materials, etc.Routinely monitor, review progress and the effectiveness of treatment plans and services, working with the team to modify interventions that are not workingIdentify and provide needed resources and linkagesFollow up on referrals, linkages, activities and outcomes to ensure client needs are metWith the team, develop transition plan that supports long-term stabilityComplete and collect signatures of all attendees of CFT meetings, provide copies of MatrixEnsure CFT meeting maintains fidelity to the ICPMBuild consensus within the team around collaborative plansEnsure the family voice and choice is heard throughout the entire teaming processEnsure all identified services are accessed, coordinated and delivered in a strength-based, individualized, family/youth driven, culturally and linguistically competent mannerActively participate in scheduled staff meetings, agency wide meetings, and other meetings as assignedSensitive and responsive to the cultural differences among the agency’s client populationActively participate in quality improvement projectsAdheres to all applicable Federal and State regulations for confidentiality, HIPAA, mandated reporting of suspected child or elder abuseFollow and adhere to all field safety guidelines.Drive personal vehicle as necessaryThe Intensive Care Facilitator will take on special projects or additional duties as assigned by the Program Lead or Program Manager or AVP of ICBSCompetency Requirements-Ability to speak, read, write and understand English-Bilingual (English/Spanish) required-Ability to work effectively as a member of the treatment team-Ability to successfully work with families of diverse ethnic and cultural backgrounds-Must have excellent oral and written communication skills-Skilled in relationship building, conflict mediation and facilitation a plus-Must have strong organizational and planning skills-Knowledge of social services system and community resources-Must have excellent interpersonal skills-Demonstrate skill in operating Microsoft Word, Outlook and ExcelEducation and Experience-Bachelor’s degree in Social Work, Sociology, Psychology or related field-1 year of relevant experience-Experience in crisis management preferred-Experience with DMH services preferredCertifications/Licenses-Valid CA driver’s license-Valid automobile insuranceAdditional Requirements-Tuberculosis clearance-Department of Justice/Fingerprint clearance-Satisfactory completion of health screening-Proof of legal eligibility to work in the United States-Satisfactory driving recordDelegates Authority ToN/APhysical DemandsWhile performing the duties of an Intensive Care Facilitator, the employee is frequently required to walk, drive, sit, and use their fingers to manipulate a keyboard or writing instrument. The employee is occasionally required to stand and reach. The employee is rarely required to run, bend, kneel and lift more than 15 pounds.Work EnvironmentThe work environment is indoors and outdoors. The noise level varies from moderate to loud.
