HUD-VASH Case Manager

The HUD-VASH Case manager is a critical position within the Lexington VA Health Care System. Under the direction of the Homeless Program manager and Chief of Mental Health, this case manager will coordinate and provide intensive clinical case management services to move veterans and their families out of homelessness. The HUD-VASH case manager has experience that demonstrates possession of advanced independent practice skills. The primary component of this position is to provide on-going clinical case management services, designed to improve veteran's health and mental health, enhancing their ability to remain stable, housed and community-integrated.

Duties include, but are not limited to:

• Responsible for direct linkages between the veteran and the local housing authority and is responsible for direct referrals for Section 8 housing vouchers as it pertains to the HUD-VASH program.
• Works closely with the VA medical center's homeless programs, VA residential and outpatient services, community care programs and transitional living facilities to ensure that coordination between these entities is facilitated.
• Provides and facilitates veterans' direct care with referral and follow-up being contiguous and based on a continuum of services.
• Independently provide clinical psychological case management services at an advanced practice level for eligible veterans and their family members in the program.
• Must make independent decisions and design policy and procedures that allow them to develop a system of care delivery in a rural and urban environment.
• Provide treatment through a well-defined program which includes clinical assessment, supportive therapeutic supervision clinical case management, medical consultation, bio-psychosocial treatment and rehabilitation, supportive group and or milieu therapy, psycho-education services, occasional vocational counseling and rehabilitation, substance abuse counseling, and socialization.
• Responsible for data collection, administrative record keeping and monitoring patient or program clinical indicators and performance measurements.
• Implements and maintains referral and screening procedures for potential HUD-VASH veteran participants that meet national and local policies.
• Conducts and participates in activities including field interviews, assessment and referrals for homeless veterans contacted in the community, shelters, and "on the streets."
• Maintains current and establishes new relationships with other community-based service providers for the homeless, and when appropriate, represents VA at community service provider meetings.
• Evaluates for admission to HUD-VASH; the veteran's situation, abilities, capabilities, and assesses for vulnerability and prioritization.

Work Schedule: Monday - Friday, 8:00 am - 4:30 pm

Pay: Competitive salary and regular salary increases.

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior work experience or military service experience.

Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66

Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA
