The Sycamore of River Falls is a growing, full-service senior living community with 84 apartments including 68 blended independent and assisted living and 16 memory care apartments in a secured neighborhood on first floor. The property has four villa patio twin home units with two bedroom and three bedroom options for active, independent seniors. The apartment building consists of a variety of studios, one-bedroom, one-bedroom plus den, and two-bedroom units. The underground parking garage has 50 parking stalls, maintenance rooms, and residents' storage area. Managed by Ebenezer. This newer care facility is Located in River Falls, WI.

Building new community, and this role will support the new Memory Care stand alone Building!

We are hiring an Assistant Director of Health Services, RN Nursing position is responsible for ensuring that the safety and health needs of the residents/clients are met while providing staff leadership duties. Supported by DHS RN, Regional Nurse, Triage Nurse and Resident Assitants/Medication Passers.

Nursing work schedule will be Monday - Friday 8am-5pm

Responsibilities

• Maintains current assessment and documentation of client's health status.
• Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensures RN Assessment is completed upon admission, within 14-days of admission, and at least every 90 days and when/if there is a change in health condition.
• Works with client and/or responsible party through collaboration, education and timely communication to ensure appropriate service and placement.
• Assures that clients have up-to-date service agreements. Implements service agreements with new tenant/residents. Reviews and updates service agreements at least once per year and more frequently when there is a change in service needs.
• Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST.
• Maintains clinical charts and progress notes using computerized documentation system.
• Communicates changes in client's conditions to medical and other relevant individuals.
• Coordinates client health services.
• Facilitates coordination of services with internal and external providers.
• Assures that all physician orders are signed and implemented per the applicable Minnesota Assisted Living regulations.
• Works with client and/or representative(s) to assure delivery of appropriate services and funding.
• Participates in Care Conferences as needed or requested by the DHS or Executive Director

Required Qualifications

• Vocational School /Technical Training or
• Associates RN Degree in Nursing
• One to Three Years Experience
• MN Registered Nurse (RN) License
• Basic Life Support (American Heart Assoc or Red Cross)

Preferred Qualifications

• Bachelor of Science Nursing
• Three to Five Years Experience
• Prior supervisory experience
• Prior experience in home care or assisted living environment
