Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Home Health experience required
• HCHB experience required

Assignment Details

• Monday through Friday, 8:00 AM to 5:00 PM
• Productivity requirement: minimum of 30 per week
• Mileage reimbursement provided
• Coverage area includes Mesa County and Garfield County
