Social Worker, Case Management, LMSW

Social Worker will be Lic. MSW in Texas, with at least 3+ years of experience. Social Worker will will be responsible for:

• Performing discharge assessments on the floor and in the Emergency Department
• Identifying High-Risk patients and assessing their potential needs
• Working with a multi-disciplinary team within hospital and outside of hospital to secure services and referrals for patients
• Following up with transitions patients who may readmit to the hospital and completing a discharge assessment, including a readmission assessment
• Communicating daily via email, phone, or in person with the Transitions of Care NP and follow up with patients who may be in-house or in the emergency department
• In addition to seeing transitions patients, this Community Connections Social Worker (CC-SW) will see patients in the Emergency Department daily that have been identified as in need of service. This can be a referral from the physician, nurse, UR nurse, or the patient and/or patient's family or the CC-SW own review of records.

Reports to: Case Management Director

MUST HAVE GENERAL ACUTE CARE HOSPITAL EXPERIENCE..IT IS A MUST
