LPN/LVN Rehabilitation (Rehab)

If you are passionate about helping patients regain independence after illness or injury, join us as a LPN/LVN in a Rehabilitation (Rehab) setting. In this role, you will provide direct patient care while supporting their recovery journey under the supervision of an RN or physician.

Key Responsibilities:

• Provide basic nursing care such as administering medications, monitoring vital signs, and assisting with mobility exercises tailored to each patient's rehabilitation goals.
• Document patient progress accurately and report any changes in condition to the supervising healthcare team.
• Assist patients with daily activities such as bathing, dressing, and eating while promoting independence whenever possible.
• Educate patients and families on post-discharge care plans and techniques for maintaining progress at home.
• Collaborate with physical therapists, occupational therapists, and other healthcare professionals to implement comprehensive care plans for rehabilitation patients.

Work Environment:

• The role is based in rehabilitation centers or hospital rehab units where LPNs/LVNs work closely with patients recovering from surgeries, injuries, or chronic conditions requiring long-term therapy support.
• This position requires patience, compassion, and physical stamina as it involves assisting patients through challenging recovery processes.
