- At least 1 year of geriatric experience
- OR LPN background
- Experience in settings such as:
- Nursing homes
- Home health
- ADvantage Case Management experience not required ; training provided
- Ability to work independently
- Strong computer and typing skills

- Case management responsibilities:
- Initiate and monitor service plans
- Establish achievable goals
- Provide resource referrals
- Meet the needs of ADvantage Program Members
