Physical Therapist - Travel Contract

Physical Therapist — Home Health — Rock Hill, SC

Join a Home Health team in Rock Hill, SC providing care for adult patients with chronic disease management and complex wound and infusion needs. The role focuses on patient-centered rehabilitation, assessment, and education in home settings across York and Lancaster counties. Clinicians will use Homecare Homebase for documentation and work with departmental leadership and interdisciplinary teams to support patient outcomes.

Minimum Qualifications

• Doctoral, Master’s, or Bachelor’s degree in Physical Therapy from an accredited program approved by the APTA.
• Active South Carolina physical therapy state license required prior to start of assignment.
• Current BLS certification required.
• Minimum 1 year of physical therapy experience; first-time travelers considered if experienced in home health.
• Required experience with Homecare Homebase charting and OASIS-E assessments.
• Experience treating adult and geriatric populations, chronic disease management, wound care, wound vac, IV/pleurx care, and maintenance notes.
• Valid driver’s license and proof of insurance required.

Preferred Qualifications

• Prior home health therapy experience preferred.
• Familiarity with Samsung Note devices and mobile documentation workflows preferred.

Assignment Details

• Shift hours: 0800 - 1700.
• Standard hours per week: 40.
• Typical daily visits: approximately 6 total points per day.
• Orientation: 5 days of New Employee Orientation.
• May be asked to float into neighboring counties (Union or Mecklenburg) on occasion.
• Scrub color: Grey - Therapy.
