Primary Care Nurse Practitioner Opportunity - Bronx, New York

Location: Bronx, New York 10459

Position: Nurse Practitioner - Family Practice (1 opening)

Deliver comprehensive primary care in the Bronx, managing acute/chronic conditions for diverse families in a community health setting.

Key Responsibilities:

Conduct physicals, vaccinations, sick visits, and wellness screenings
Manage diabetes, hypertension, asthma, and preventive care
Order labs/imaging, prescribe medications, and coordinate referrals
Provide patient education and chronic disease counseling

Qualifications:

FNP certification with active NY NP license
Primary care experience preferred; bilingual Spanish advantageous
BLS; EHR proficiency required
