Join a community health center in Kansas City as a Nurse Practitioner or Physician Assistant delivering Patient Centered Medical Home primary care, earning $104,536–$130,670 annually. You will have a real voice in how care is delivered, participating in Medical Executive meetings, quality improvement work, peer review, and pharmacy and therapeutics committees alongside senior clinical leadership. What You'll Do Provide acute, chronic, and preventive primary care with emphasis on chronic disease management and pediatrics Review and interpret lab, radiology, and consultation reports and direct follow-up accordingly Coordinate referrals to internal support services and outside partners to remove barriers to care Participate in Medical Executive meetings, quality improvement, peer review, and committee work Collaborate with the CMO and Chief Clinical Officer on service evaluation and program development Maintain thorough, timely documentation in accordance with HIPAA and organizational standards Stay current with evidence-based practice through ongoing literature review Qualifications Active, unrestricted Missouri FNP or PA-C license in hand at application (hard requirement; no exceptions) FNP-BC or PA-C certification required Graduate of accredited NP or PA program BLS certification requiredCommitment to PCMH model and collaborative, committee-based care Active Missouri DEA and BNDD licensure preferred Interest in chronic disease management and pediatrics preferred Compensation and Benefits $104,536–$130,670 annual salary Medical, dental, and vision insurance Malpractice insurance covered Continuing education assistance #J-18808-Ljbffr NPHire
