CODA, Inc. | Opioid Treatment Program (OTP) Full-Time | Exempt | Monday–Friday (Day Shift)

What You'll Be Doing

You will play a key role in coordinating care and improving patient outcomes through both direct engagement and data-driven decision making:

• Track patient progress during the first 90 days of treatment and intervene as needed
• Conduct proactive outreach to patients (calls, letters, follow-ups)
• Coordinate care with internal teams and external partners (jails, SNFs, community providers)
• Monitor key data points such as missed doses, UAs, and treatment adherence
• Support Medication First Track patients and assist with panel management
• Participate in interdisciplinary team meetings, huddles, and care planning
• Coordinate referrals for housing, primary care, mental health, and other services
• Maintain accurate documentation in the electronic medical record
• Ensure compliance with regulatory standards (OARs, Joint Commission, SAMHSA)
• Support development of dashboards and data tracking to improve outcomes

Qualifications

What You Bring

• 2+ years experience in care coordination, healthcare, or social services
• Strong understanding of community resources and patient engagement strategies
• Ability to analyze data and translate it into action
• Strong communication and relationship-building skills

Certifications (Required or Within Timeline):

• CADC I (or ability to obtain within 24 months; CADC-R within 30 days minimum
• CPR Certification (within 90 days)
• Crisis Prevention Training (within 90 days)
• Valid OR/WA Driver's License (if required for role)
