(QHCG) is a CARF-accredited human services organization based in Delaware County, Pennsylvania, providing home health and community-based services to individuals across the lifespan. Our services span Title 28 home health (Chapters 601/611), residential habilitation, and intellectual/developmental disability (IDD) support under Title 55 (Chapters 6100/6400), as well as OLTL/Community HealthChoices waiver programs. Position Summary The House Manager at QHCG is the on-site operational lead for a single assigned residential habilitation home serving individuals with intellectual and developmental disabilities. This individual is present across shifts and holds direct accountability for the home environment, resident wellbeing, ISP implementation, documentation integrity, and the daily performance of all Direct Support Professionals assigned to the home. The House Manager bridges direct care and program management ensuring that each resident receives consistent, person-centered support in a safe, structured, and home-like environment, while maintaining full regulatory compliance with Pennsylvania ODP Title 55 (Chapters 6100/6400) requirements. This is not an entry-level position. The ideal candidate is a reliable, experienced human services professional with direct residential habilitation experience and a natural leadership presence. The House Manager reports directly to the Residential Program Supervisor or Administrator/Associate Administrator. Key ResponsibilitiesResident-Centered Care & Individual Programming Maintain thorough, current knowledge of every resident's ISP, care plan, behavioral support plan, diagnosis, history, and primary habilitation goals Implement ISPs and Behavior Support Plans (BSPs) consistently and correctly across all shifts; coach and correct DSP staff when implementation lapses are observed Build and publish a monthly activity calendar for the home aligned to resident ISP goals, posted a minimum of two weeks in advance; coordinate a minimum of four community integration outings per month Implement structured daily and weekly routines covering meals, hygiene, personal care, recreation, vocational skill building, and life skills Monitor individual progress toward ISP goals and communicate updates to the Residential Program Supervisor, program specialist, behavior specialist, and clinical team Coordinate medical appointments, transportation, and community integration activities for all individuals in the home Participate in ISP meetings, quarterly reviews, and interdisciplinary team conferences; document and report progress toward active habilitation goals Uphold and document resident rights in compliance with ODP/OLTL standards; address, document, and resolve grievances per QHCG policy Communicate with residents' families on a regular basis to provide updates on programming, health status, and goal progress Serve as the home-level point of contact for Support Coordinator inquiries regarding the individuals served Staff Supervision & Development Directly supervise, coach, and evaluate all Direct Support Professionals (DSPs) and Home Health Aides (HHAs) assigned to the home across all shifts Conduct documented observed shift evaluations for each DSP at least monthly; provide written feedback and document findings using QHCG evaluation forms Complete all staff competency evaluations using the QHCG competency checklist; maintain a current training due-date matrix covering CPR, Medication Administration, OSHA safety, and abuse/neglect certifications Administer and document corrective counseling for attendance and performance issues using QHCG HR forms; escalate to the Residential Program Supervisor or administration when appropriate Conduct on-site orientation and skills-based training for all new DSP staff assigned to the home Conduct mid-probation check-ins for all staff within their first 90 days Foster a team culture rooted in accountability, respect, and person-centered values Work closely with the Residential Program Supervisor to problem-solve staffing, scheduling, and performance issues across shifts Scheduling & Staffing Management Publish and maintain a two-week rolling schedule for the home on time, every pay period Implement and follow a shift-fill protocol for call-outs with documented escalation steps; notify the Residential Program Supervisor when coverage cannot be resolved at the home level Monitor and proactively flag overtime before it is incurred; adjust coverage to protect budgeted hours and staffing ratios Ensure 1:1 and 1:2 staffing ratios are maintained per each resident's active service authorization at all times Identify chronically absent staff and initiate documentation or counseling processes in line with QHCG HR policy Payroll & EVV Oversight Review and submit accurate timecards each pay period; identify and correct missed punches before submission deadline; escalate unresolvable discrepancies to the Residential Program Supervisor Reconcile EVV clock-in/out data against scheduled shifts each pay period; resolve all discrepancies before payroll submission Track PTO usage and balances for all home staff; flag issues to the Residential Program Supervisor promptly Maintain an ongoing payroll discrepancy log available for audit review at any time Regulatory Compliance & Documentation Ensure the home operates in full compliance with Pennsylvania Title 55 Chapters 6100 and 6400 at all times Ensure all daily Therap and EIM documentation is completed within 24 hours with no gaps; conduct monthly self-audits for timeliness, narrative quality, and accuracy Audit all Medication Administration Records (MARs) monthly for completeness and accuracy; zero missed signatures or documentation gaps are acceptable Monitor all residents' medical, physical, dental, vision, and psychiatric appointment compliance; coordinate the scheduling and documentation of all required appointments in Therap per Chapter 6400 timelines File all incident reports within 24 hours of any reportable event via HCSIS, applicable MCO portals, and internal QHCG forms; notify the Residential Program Supervisor immediately upon any reportable event Verify all residents have current physicals, dental, and vision appointments on file within required regulatory timeframes Confirm fire drill logs are current, completed monthly, documented, and signed by all staff Maintain and reconcile the petty cash and resident funds ledger; submit a balanced, documented report to the Residential Program Supervisor for quarterly audit Prepare the home for ODP licensing inspections, CARF accreditation reviews, and MCO audits under the direction of the Residential Program Supervisor Physical Site Inspections & Safety Conduct monthly home inspections covering fire and life safety, interior environment, resident rooms, health and medical readiness, documentation compliance, and exterior/vehicle using QHCG's standardized inspection checklist Identify and report all maintenance, safety, or environmental deficiencies immediately to the Residential Program Supervisor; track corrective action through resolution Verify fire extinguisher inspection dates, smoke detector function, egress paths, and posted emergency evacuation plans on a monthly basis Confirm vehicle registration, inspection sticker, and insurance are current where applicable Submit completed monthly inspection reports to the Residential Program Supervisor for administrative review and filing Quality Assurance & Reporting Submit all required monthly compliance reports to the Residential Program Supervisor: incident summary, staffing ratio logs, and medication administration records Track and report ISP goal progress data for all residents on a quarterly basis to the Residential Program Supervisor and interdisciplinary team Identify and communicate operational concerns, documentation gaps, or resident support issues to the Residential Program Supervisor promptly and in writing Support QAPI data collection at the home level by maintaining accurate and timely documentation, incident logs, and activity records Required Qualifications High school diploma or GED required; Associate's or Bachelor's degree in Human Services, Psychology, Social Work, or a related field preferred Minimum 23 years of experience in a residential habilitation, group home, or IDD direct care setting Minimum 1 year of supervisory or lead staff experience over direct care staff in a licensed residential program Working knowledge of Pennsylvania ODP regulations (Title 55, Chapters 6100/6400) and person-centered planning Experience with medication administration and MAR documentation in a residential setting Valid Pennsylvania driver's license with a clean driving record Ability to pass Pennsylvania criminal background check, child abuse history clearance, and FBI fingerprinting as required under applicable regulations Current CPR/BLS certification (or willingness to obtain prior to start) Strong written and verbal communication skills; ability to maintain professional documentation and communicate effectively with families, guardians, support coordinators, and clinical staff Preferred Qualifications Prior experience as a House Manager, Residential Lead, or Program Lead in a Chapter 6100 or 6400 licensed residential setting Familiarity with Therap EHR platform for documentation and individual records management Experience implementing ISPs and behavior support plans developed by a behavior specialist Knowledge of OLTL/Community HealthChoices waiver programs and MCO support coordination processes Experience with EVV platforms such as HHAeXchange or Sandata Experience with UKG or a comparable workforce management platform for scheduling NADSP E-Badge certification or Frontline Supervisor training completion Bilingual candidates strongly encouraged to apply What Success Looks Like in This Role Within 30 days, this person knows every resident by name, diagnosis, and active ISP goals; has completed all required onboarding training; has submitted an initial home safety inspection using the QHCG checklist; and has reviewed and confirmed accuracy of all active staff schedules and EVV configurations. Within 60 days, this person independently publishes schedules each pay period, owns the timecard and EVV reconciliation process without prompting, has initiated competency evaluations for all DSP staff with at least 50% completed, and has conducted documented observed shift evaluations for each assigned staff member. Within 90 days, this person runs the assigned home without daily oversight, has completed all DSP competency evaluations, has passed the 90-day performance review with role readiness confirmed, and has submitted the first monthly compliance reports incident summary, staffing ratio log, and MAR audit to the Residential Program Supervisor on time.
