Job Description Job Description Description of the role: We are seeking a Licensed Practical Nurse (LPN) to join our team at Pleasant Hill Adult Day Health Center and Senior Services. In this role, you will provide essential nursing care to our residents/clients, ensuring their health and well-being while supporting their journey. Schedule: Monday - Friday: 9:00 AM - 5:00 PM One weekend per month (Saturday and Sunday) 8:00 AM - 8:00 PM Responsibilities: Administer medications and treatments as prescribed by physicians. Perform triage assessments and monitor patient vital signs. Collaborate with healthcare team members to develop and implement patient care plans. Educate residents and families on disease processes. Maintain accurate patient records. Supervisory visits in clients' homes regarding Certified Nursing Assistants/Personal Care Technicians. Requirements: Current LPN license in the state of practice. Proven experience in a skilled nursing setting preferred. Strong communication skills for effective interaction with patients and families. Ability to perform clinical procedures competently. Compassionate demeanor with a commitment to patient-centered care. Benefits: We offer competitive compensation ranging from $25.00 to $29.00 per week, paid bi-weekly.
