At Concho Hearts Hospice, our On-Call Nurses are there when patients and families need support the most. Whether providing symptom management, answering urgent questions, or offering reassurance during difficult moments, our nurses play a vital role in delivering compassionate end-of-life care. We are seeking a dedicated RN to join our Austin team and provide after-hours support to hospice patients and families.

What You'll Do:

• Respond to after-hours patient and family calls with professionalism and compassion.
• Assess patient needs and provide appropriate nursing interventions.
• Complete urgent visits in patients' homes or facilities as needed.
• Provide symptom management and end-of-life support.
• Communicate patient status changes with the clinical team and physicians.
• Educate and support patients, caregivers, and families.
• Complete timely and accurate documentation in the EMR system.

What We're Looking For:

• Active Texas RN License.
• Hospice, home health, long-term care, or acute care experience preferred.
• Strong assessment and critical thinking skills.
• Ability to work independently and make sound clinical decisions.
• Excellent communication and customer service skills.
• Reliable transportation and willingness to travel throughout the service area.
• Current CPR certification.

Hospice experience is preferred but not required. We are willing to train the right nurse who has a passion for compassionate patient care.
