We are hiring an Admission Registered Nurse (RN) for a Hospice position providing end-of-life care to patients in their homes in the Scranton, PA area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year! What You’ll Do: Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life Conduct comprehensive hospice admission assessments, including clinical evaluations, medication reconciliation, and development of individualized plans of care aligned with physician orders Educate and guide patients and families through the hospice admission process, setting expectations, explaining the hospice philosophy, and ensuring they feel informed, supported, and comfortable with the transition to end-of-life care Apply strong clinical judgment and compassionate critical thinking to facilitate timely admissions, address immediate needs, and ensure accurate, thorough, and compliant EMR documentation Initiate evaluations to other disciplines when indicated Work independently, but never alone Travel to patient homes within assigned territory Designed for Success Dedicated field education team supporting you from Day 1 Leadership that listens, mentors, and champions your growth A collaborative culture where your impact is recognized and your feedback shapes improvement Benefits That Support Your Whole Life Medical, Dental, Vision, and Pet Insurance Fleet Vehicle Program Generous PTO Pension + 401K Paid parental leave Up to 90% off degrees and certifications through our Purdue University partnership Admission RN Qualifications Active Registered Nurse, RN license in the state you work Two years of RN experience and the clinical expertise to deliver quality patient care, hospice experience preferred Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required We are an equal opportunity employer and value diversity at our company.
