Case Management Program Coordinator

Provides excellent patient care by assisting in the collaboration, development, implementation, revision and reporting of the case management program. Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient's healthcare benefactor to optimize outcomes. Serves as a consultant to the healthcare team on specific patient items. Coordinates a continuum of care for a defined population of patients from pre-admission through post discharge. Assures that patients and families proceed efficiently through the course of hospitalization.

This position is for an EPIC conversion. Candidates must have EPIC and Cerner experience within the last 5 years and have worked as an employee of an organization / successfully completed a contract. Each work history entry must list the charting system used. If multiple EMR systems are listed on one job, dates must be provided to clarify when each system was used. No RTO is allowed. The listed start date is firm. Auto Offer EPIC BACKFILL, Day Shift - 40 hours per week (8x5's).
