Registered Dietitian (RD) PRN

Job Summary:

We are seeking a PRN Registered Dietitian to join our team at Rosewood Gardens Rehabilitation and Nursing Center, a 146-bed skilled rehabilitation and nursing facility located in Darby, PA. As a PRN Registered Dietitian, you will be responsible for providing nutrition services to our residents, including nutrition screening, assessment, diagnosis, intervention, monitoring, and evaluation of resident care plans.

About the Role:

As a Registered Dietitian at Rosewood Gardens, you will work closely with our interdisciplinary team to develop and implement individualized nutrition care plans, monitor and revise nutrition interventions to ensure desired outcomes, and maintain accurate documentation within our electronic medical record (EMR). We are seeking a highly motivated and experienced Registered Dietitian with a strong background in long-term care and experience with MDS, PDPM, and NFPE.

Responsibilities:

Complete required nutrition assessments per MDS schedule
Develop and implement individualized nutrition care plans
Monitor and revise nutrition interventions to ensure desired outcomes
Maintain accurate documentation within our electronic medical record (EMR)
Collaborate with the interdisciplinary team to provide comprehensive care to our residents

Requirements:

Bachelor's degree in nutrition and Dietetics from an accredited program
Active Registered Dietitian (RD/RDN) credential in good standing
Experience with MDS, PDPM, and NFPE
Proficiency in PointClickCare or similar EMR preferred
Long-term care experience strongly preferred (2+ years ideal, but less experience considered)
