Registered Nurse (RN), Per Diem, Home Care Field Visits

Senior Helpers of North Hudson is a veteran owned home care agency serving North Bergen, Union City, West New York, Weehawken, Guttenberg, Hoboken and Jersey City. We are looking for a per diem RN to handle our in home client visits.

What We Offer

• $90 per initial assessment visit (start of care)
• $55 per supervisory and 60 day case monitoring visit
• Per diem and part time. Flexible schedule, you take the visits that fit your week
• Mileage reimbursed at the IRS standard rate, currently 76 cents per mile, for travel between client visits
• Paid every other Friday by direct deposit
• A compact Hudson County territory, so less time driving between visits
• Office support Monday to Friday, 9am to 5pm. You are never on your own in the field
• Fully digital charting. No paper files

What You Will Do

• Complete initial client assessments in the home and write the plan of care
• Perform supervisory and case monitoring visits every 60 days
• Orient home health aides to the case and confirm they can carry out the plan of care
• Complete aide clinical competency evaluations
• Reassess clients when their condition or needs change and update the plan of care
• Communicate changes to the family and to the office
• Document each visit the same day in our electronic record

What We Are Looking For

• Current New Jersey RN license in good standing
• Spanish and English. Many of our clients, families and aides speak Spanish
• Home care, home health or community nursing experience preferred
• Valid driver license, reliable transportation and current auto insurance
• Comfortable working independently and charting on a laptop or tablet
