Medical Social Worker - Care Coordination - PRN Days

Job Summary
Utilizes professional social work skills in collaborating with other members of the medical team to meet the needs of patients and works with patients (either individually or in groups) to prevent or ameliorate related social problems.
Responsibilities:
Intervention/Planning
Coordination/Implementation
Regularly cross-trains health system wide to sustain proficiency across multiple Social Services functional areas.
Assess each patient, identify discharge planning needs and initiate a discharge assessment. Additionally, address and document management of social determinants of health.
Identify psychosocial needs of patients and families including social determinants of health. When possible, adhere to cultural, religious practices and make adjustments for any special impairment.
Discuss the home situation with patient/family/caregiver as needed. Coordinate homecare as needed. Discuss with the team to coordinate the best plan and identify any barriers to discharge.
Additional Duties as Assigned
Qualifications:
Masters of Social Work required
Previous experience providing social work/case management services preferred
Experience in a hospital environment preferred
