You’ll collaborate with a care team to develop a personalized plan of care, lead meaningful end of life conversations, and ensure exceptional symptom management with empathy and clinical excellence.If you're a compassionate Registered Nurse who values meaningful patient connections and autonomy in your practice, we’d love to meet you.This role will be based out of our Palos Heights branch office but will be assigned territory in the Kankakee/Momence/Bourbonaiss/Bradley area.Registered Nurse Case Manager Requirements & Responsibilities:Associates, Bachelors, or Masters of an accredited nursing program (RN)1+ years of recent acute care experience required; hospice/home health experience preferredComplete initial and ongoing comprehensive assessments to determine hospice needsDevelop individualized care plans with the patient and family, updating as needs evolveAdminister medications and treatments as prescribedEducate patients and caregivers on care techniques, medications, diet, and symptom managementRotate on-call duties and ensure seamless care transitionsMust have reliable transportation, valid driver’s license, and proof of auto insuranceBenefits & Schedule:Schedule: 8:00am to 5:00pm, Full-time position with on-call expectationsHealth, dental, vision, and life insuranceCompany-paid short/long-term disability and basic life insurance401(k) with up to 4% company matchTuition reimbursement and professional development opportunitiesPaid time off and paid holidaysThe starting salary range for this role is $85,000 to $97,000. An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets.
