Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 hours Shift: 2x3 days Hours per Shift: 12 hours Experience: Two years in a home health setting within the last fifteen years License: Current RN Licensure in Client Certifications: BLS required before start date Must-Have: - Recent experience conducting comprehensive assessments of patients - Ability to work with technology in the workplace - Knowledge of the theory, principles, and practices of professional nursing - Knowledge of state and agency laws and regulations governing nursing practices - Excellent organizational, interpersonal, written, and verbal communication skills - Ability to communicate and work effectively with consumers of various backgrounds - Strong ability to work independently - Ability to perform comfortably in a fast-paced, deadline-oriented work environment Description: This position requires the ability to review health status questions and make evaluations based on criteria for enrollment. Nurses perform initial in-home pre-evaluations of patients that include assessing health status, strengths, care needs, and preferences. They review consumer medical documentation, entering evaluation data into electronic forms. Documentation of concerns and conflicting information during the evaluation process is essential. Continuity of care is emphasized to reduce fragmentation, duplication, and gaps in treatment plans. Nurses discuss healthcare options and service vendor choices with consumers. They initiate communication with eligible consumers for enrollment into managed care health plans. Collaboration with interdisciplinary care team members is vital to improve care coordination. Quality review data is collected to support outcome measurements and record case notes. Nurses identify opportunities for health promotion and illness prevention. Maintaining a comprehensive knowledge of community resources and network services for the target population is required. All standards for this position must be met as outlined in performance criteria. Additional duties may be assigned by the Regional Nurse Manager or project management.
