Job Description:

The Physical Therapist will provide home health services, requiring experience with Home Care Home Base and Home Health practices. The role includes attending to common diagnoses such as CHF, ortho, and pneumonia, with a focus on patient care in the home health setting.

Responsibilities:

• Conduct start of care, recertifications, evaluations, and daily visits.
• Manage a caseload to meet a daily requirement of 6.5 points.

Requirements:

• Experience with Home Care Home Base charting system.
• BLS certification (AHA required).
• Prefer candidates with prior home health experience; no new grads preferred.
