About the Role:

As a Physical Therapist, you will be responsible for developing and implementing individualized treatment plans to help our residents achieve their maximum potential. Your expertise will enable residents to regain strength, mobility, and independence, allowing them to live life to the fullest. You will work closely with our interdisciplinary team to ensure comprehensive care and deliver exceptional patient outcomes.

Responsibilities:
• Conduct initial assessments and develop personalized treatment plans for each resident
• Implement and conduct therapy sessions, including exercises, stretches, and other interventions
• Monitor and record resident progress, adjusting treatment plans as needed
• Collaborate with healthcare professionals, including physicians, occupational therapists, and speech therapists
• Communicate effectively with residents, families, and healthcare teams to ensure seamless care coordination
• Participate in team meetings and provide input on resident care and services

Requirements:

• Current Physical Therapy license in the state of Pennsylvania
• Doctorate degree in Physical Therapy or related field
• Ability to work in a fast-paced, dynamic environment
• Excellent communication and interpersonal skills
• Strong attention to detail and organizational skills
