Responsibilities:

Presbyterian Healthcare Services is seeking a passionate, community-minded Licensed Physical Therapist (PT) for our multidisciplinary rehabilitation team in Espanola, NM. In this role, you will evaluate and treat diverse pediatric to geriatric patient populations across versatile inpatient rehab and outpatient clinic settings.

As a dedicated clinical patient advocate , you will collaborate on interdisciplinary healthcare coordination with medical teams, drive progressive discharge planning goals , utilize modern EPIC EMR software , and provide caregiver training to optimize long-term patient independence.

This position is eligible for the Northern New Mexico differential of $4.00 per hour. (This differential may be adjusted or discontinued based on business requirements.)
