Case Manager

A client of Innova Solutions is immediately hiring for a MSW Case Manager

Position type: Full-time, Contract

Duration: 7+ Months

Location: Hybrid Remote (Candidate is required to pick up their laptop in Pasadena and attend one day of onsite training in Pasadena)

As a MSW Case Manager, you will:

• Exceptional Communication & Teamwork: Deliver outstanding customer service, collaborate effectively with team members, and proactively seek support when needed.
• Holistic Case Management: Approach cases with big-picture thinking, recognize emergencies, and be willing to learn medical aspects relevant to patient care.
• Care Plan Development: Create clear, actionable care plans (e.g., exercise routines, follow-up appointments) based on patient needs and chart references.
• Critical Thinking & Adaptability: Demonstrate strong listening skills, think on your feet, and ask insightful follow-up questions to guide patient conversations.
• Education Requirement: Master of Social Work (MSW) required; Licensed Clinical Social Worker (LCSW) may be accepted in lieu of MSW.
• Work Schedule: Monday to Friday, 9:00 AM - 5:30 PM PST, with flexible start times available upon request.

The ideal candidate will have:The ideal candidatewill hold an MSW or LCSW, have at least 2 years of relevant experience, strong communication and critical thinking skills, and the ability to manage cases holistically in a collaborative, remote setting.

PAY RANGE AND BENEFITS:

Pay Range*: $30 - $34 per hour

*Pay range offered to a successful candidate will be based on several factors, including the candidate's education, work experience, work location, specific job duties, certifications, etc.
