Registered Nurse Admissions We are seeking a compassionate and skilled Registered Nurse Admissions Nurse to join our dedicated team of caregivers at Van Duyn Center for Rehabilitation and Nursing in Syracuse, New York. As a key member of our admissions team, you will play a vital role in providing exceptional care and service to our residents and their families. Responsibilities: Conduct comprehensive nursing assessments to identify the needs of potential residents and develop individualized care plans Collaborate with interdisciplinary teams to develop and implement care plans that address the physical, social, emotional, and spiritual needs of residents Provide education and support to residents and their families on various aspects of care and troubleshooting Ensure adherence to regulatory standards and company protocols to maintain optimal quality of care Develop and maintain positive relationships with residents, families, and healthcare professionals to ensure a seamless transition into our care community Participate in quality improvement initiatives to continuously enhance the delivery of care and services About the Role: As a Registered Nurse Admissions Nurse at Van Duyn Center for Rehabilitation and Nursing, you will have the opportunity to make a meaningful difference in the lives of our residents and their families.
