Nurse Practitioner - Thunderbird Area

Arizona Kidney Disease and Hypertension Center (AKDHC) has provided care for patients with renal disease in the Phoenix Metropolitan area and across Arizona since 1976. It is a professional medical association consisting of physicians and other support staff. Responsibilities Collaborate with AKDHC physicians and dialysis staff to complete and document dialysis visits, addressing medical issues promptly. Provide chronic kidney disease care in office settings and coordinate treatment plans with physicians. Support care transitions, including annual physicals and post-hospital discharge planning. Deliver care across settings (rehab, SNF, office, or on call), including diagnostic evaluations and treatment planning. Prescribe or recommend appropriate therapies and maintain professional, respectful conduct in all interactions. Qualifications Master’s degree in nursing or related field, or equivalent combination of education and experience (4-10 years). Current Arizona RN license and Nurse Practitioner or Physician Assistant license. Proficiency in Microsoft Office and experience with ICD-9 and CPT coding. Solid interpersonal skills, flexible schedule availability, and reliable transportation for travel between sites.
