Case Manager Mental Health Professional (MHP), Dekalb, IL

Case Manager Mental Health Professional The Association for Individual Development (AID) seeks a Case Manager Mental Health Professional located in Dekalb, IL who will develop and implement individualized treatment plans using the IM+CANS framework to maximize clients’ independent community living skills. The role requires documentation of mental health services in accordance with state and agency standards, collaboration with a multidisciplinary behavioral health team, and adherence to all regulatory and policy requirements. Responsibilities Work scheduled hours and be flexible to meet client and program needs, as assigned by Program Manager or Director. Provide independent program coverage when scheduled. Ensure compliance with all agency, state, and federal regulations; review and follow updated policies and procedures. Facilitate communication and coordination of services with other AID staff and community professionals by phone and email. Maintain professionalism and boundaries when working with clients, coworkers, and outside agencies. Meet minimum service hour productivity standards (MRO) on a monthly basis. Complete all required case management documentation (IM+CANS, consents, residential/agency paperwork) in a timely manner. Sign all MRO documentation within 48 hours using Cx360. Meet with assigned clients regularly, submitting daily activity logs. Develop, review, and revise the IM+CANS, ensuring corrections are made within the allotted timeframe. Assist clients with activities of daily living, including apartment maintenance and training consistent with treatment goals. Provide guidance to MHP and RSA staff regarding implementation of IM+CANS goals and objectives. Gather input from clients, families, guardians, and other staff on improving residential services. Monitor and report client safety and living arrangements. Acquire and maintain required trainings and certifications. Assist clients in paying bills, budgeting, and banking per program procedure. Oversee client funds when necessary, including dual signature accounts and AID representative payees. Secure client benefits such as Social Security, Medicaid, Link Card, etc. Maintain client records in accordance with agency procedures. Complete authorizations, reauthorizations, and spend‑down paperwork promptly. Update Cx360 annually and as needed to ensure record accuracy. Provide services aligned with client rights and the Mental Health Recovery Model. Attend monthly clinical supervisions required by DHS. Participate in monthly team meetings and contribute positively. Recognize emergency situations, take appropriate action, and complete necessary paperwork (incident reports, petitions, encounters). Promote relationships among clients’ families and additional supports. Provide transportation to clients when necessary for work, shopping, appointments, etc. Coordinate services in the community to meet client needs (financial, legal, advocacy, medical, etc.). Engage in planning and implementation of client activities. Offer coaching, counseling, consultation, motivational enhancement, and information/referral services to clients facing health challenges. Communicate with service partners (physicians, nurse practitioners, counselors, pharmacists, employers, community supports, and other health professionals) to facilitate clients’ wellness objectives. Utilize resources and educational materials for wellness coaching, such as Health Matters curriculum, smoking cessation materials, and weight management information. Promote awareness and respect for client, employee, and stakeholder diversity. Perform all other related duties as assigned in accordance with AID policies and philosophies. Qualifications Bachelor’s degree in Human Services or a related field. Experience working in social services. Physical ability to navigate stairs, provide services in clients’ homes, and use a personal automobile for client transport. Valid driver’s license and minimum liability insurance as per AID Personnel Policy. Certified in First Aid, CPR, Non‑violent Crisis Intervention, and other relevant trainings; maintain IM+CANS certification. Computer proficiency with Microsoft Windows applications and basic household appliances; telephone with voicemail system. Willingness to work evening hours as required.
