• Required certification: Med/Surg
• 1+ years of experience
• Unit Notes:OSF ITEMS OF NOTE: - Local Rate: Applies to clinicians with permanent addresses within 50 miles (-$25 to regular and OT bill rates) - OSF Call Off Policy: Equivalent of 12 hours every 13 weeks with no penalty or need to pay for the shift - Non-billable Orientation Hours: 12 hours, guaranteed hours will not apply during orientation REQUIREMENTS FOR SUBMISSION: - Last four (4) digits of clinician's SSN - Clinician's middle initial - Requested time off - Active IL license or confirmati
