Licensed Clinical Social Worker (LCSW) – Neuro Community-Based CarePart-Time / PRNWe are seeking a Licensed Clinical Social Worker to support individuals and families navigating the emotional, behavioral, and life changes that come with neurological conditions.This role focuses on real-world psychosocial support delivered in home and community environments, helping patients and families adapt, stabilize, and rebuild function in daily life.You will be part of a neuro rehab team that values autonomy, flexibility, and deep clinical impact.You will focus on:Psychosocial adjustment to injury or illnessFamily and caregiver supportCare coordination and resource navigationCommunity reintegration supportWhat makes this different:Flexible scheduling with full autonomyCommunity-based practice (not office-bound)Meaningful one-on-one client relationshipsPaid hourly + documentation timeMileage reimbursement + drive time payCollaborative neuro rehab teamIf you want your clinical work to directly support real-life recovery and independence, this role offers that opportunity.Responsibilities• Conducts comprehensive evaluations, assesses the social, emotional and support needs of patients and their families, and sets measurable objectives and goals that are formulated in conjunction with the rehabilitation team. Adjusts treatment plan to achieve maximum results in independence and/or function.• Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives. Utilizes special skills and judgment to resolve adverse social and emotional responses connected with the injury/illness.• Provides direct social work intervention and instructs and counsels the patients and family members in treating and coping with social and emotional responses connected with the injury/illness.• Regularly re-assesses effectiveness of treatment plan, progress of patient towards goals and the need for modifying goals or treatment.• Communicates patient's needs and progress to the treatment team, physician, patient, family members, and others as applicable.• Consults with medical providers regarding patient evaluation & treatment, progress of the patient & discharge from treatment.• Observes/documents and reports changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other treatment team members.• Maintains and submits evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations.• Adheres to standards of the Company's Code of Business Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.• Performs other duties as assigned.Qualifications• Master's Degree in Social Work or Marriage and Family Therapy (LCSW, MSW, MFCC, or LPC)• Current Social Work or Marriage and Family Therapist licensure/certification in the state(s) of practice required by individual states.• A minimum of one year of experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or other acute diagnosis.• Current BLS (Basic Life Support) First Aid and CPR certifications.• Demonstrates effective and professional interpersonal, verbal, and written communication skills.• Able to work independently and part of a team.• Demonstrates strong organization skills and attention to detail.Salary RangeUSD $44.00 - $48.00 / Hour
