Hospice Acute Care NP - Hybrid Inpatient/Community

Northwestern Medicine has an exciting opportunity for an Acute Care Nurse Practitioner to work with our Hospice team in a hybrid inpatient/outpatient role. The APP will provide comprehensive hospice and palliative care services across inpatient and community settings. Schedule: Monday-Friday, 11:00am-3:00pm with on-call rotation including nights, weekends, and holidays approximately every 13 weeks.
