Why This Role Matters As a Home Health Physical Therapist, you are a critical part of the care team - helping patients safely recover, regain mobility, and maintain independence where they feel most comfortable: at home. This is a highly autonomous, relationship-driven role where you can truly see the impact of your work every day. What You'll Do Deliver Patient-Centered Care * Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential * Develop and implement individualized care plans in collaboration with physicians and the care team * Provide hands-on therapy to improve mobility, strength, balance, and independence * Educate patients and caregivers on treatment plans, exercises, and safety strategies Drive Outcomes & Recovery * Monitor patient progress and adjust treatment plans as needed * Support safe transitions of care and contribute to discharge planning * Identify additional patient needs and collaborate with interdisciplinary teams Collaborate & Communicate * Partner closely with nurses, therapists, and physicians to ensure coordinated care * Document patient care and progress accurately and timely within the EMR * Participate in case conferences and team meetings Lead & Support * Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines * Provide guidance and education to caregivers and support staff What You Bring * Graduate of an accredited Physical Therapy program (DPT or Master's) * Current PT license in the state of practice * Minimum 1-2 years of experience (home health experience preferred) * Strong clinical assessment and critical thinking skills * Excellent communication and patient education abilities * Current CPR certification * Valid driver's license, reliable transportation, and auto insurance What Sets You Apart * You value autonomy and thrive in a field-based environment * You're passionate about patient independence and functional outcomes * You build strong, trusting relationships with patients and families * You bring both clinical excellence and compassionate care to every visit What We Offer Health & Wellness * Medical, Dental & Vision coverage * Pharmacy benefits * Virtual care & mental health support * FSA & HSA options * Supplemental health & life insurance Financial & Protection * 401(k) with company match * Competitive compensation + mileage reimbursement * Employee referral program * Prepaid legal services & identity theft protection Work-Life Balance & Growth * Flexible scheduling & autonomy in your day * Generous Paid Time Off * Ongoing clinical education & career advancement opportunities Care That Transforms Lives -- Including Yours At VitalCaring, every visit matters.
