RN Case Manager: Discharge & Care Coordination

UF Health

UF Health is seeking a Case Manager to participate in patient care planning, facilitate throughput and discharge planning, and perform utilization review in collaboration with the care team. The role emphasizes strong assessment, communication, and coordination with physicians, patients, and families to ensure safe transitions. Qualified candidates will have Florida RN licensure and the ability to obtain a BSN within three years of hire, with 1–2 years in discharge planning or case management as #J-18808-Ljbffr
