Case Manager MSW

What We Offer
As a Case Manager MSW at Novant Health Presbyterian Medical Center, you will serve as a leader of the complex discharge planning process. Your work will support admissions and discharge planning for patients on our 12 bed skilled nursing unit . Presbyterian Medical Center is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte.
This position offers full time benefits eligibility status, while providing the work/life balance of a part time schedule. What You'll Do
- Serves as a leader of the complex discharge planning process and a valued member of the healthcare team
- Care plans for SNU admissions
- Support admissions for SNU unit
- NOMNC forms
- Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive, complex and safe discharge plans
- Responsible for facilitating coordination of care
- Accepts responsibility, authority and accountability for management of care in accordance with the current policies and procedures, mission, vision, and values of Novant Health
What We're Looking For
- Education : Master's Degree in Social Work from school accredited by the Council on Social Work Education, required.
- Experience : Two or more years professional social work experience or two or more years healthcare experience, preferred. Case Management or related experience, preferred. Experience in a skilled nursing facility is a plus.
- Additional Skills (required): Weekend coverage.
- Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding.
