Physical Therapist (PT) - Outpatient Rehab - Ortho - Marietta, GA

Job Summary The physical therapist assesses, plans, organizes and participates in rehabilitative programs that improve mobility, relieve pain, increase strength and decrease or prevent deformity of patients suffering from disease or injury. Services are provided across the full continuum and rendered in a cost conscious, quality focused and customer oriented manner. Compliance with all applicable regulatory standards is also required in this role. Core Responsibilities And Essential Functions Implement the physical therapy treatment plan. Select appropriate treatment activities to progress patients toward goals. Revise goals and plan of care with patient/family input. Provide ongoing patient/family education and training. Provide discharge instructions, follow-up, and referral to community resources. Act as a referral source for staff, families, physicians on services and equipment related to rehabilitation services. Collaborate with physicians to establish and evaluate treatment programs, plan of care, goals, and discharge plans. Create realistic discharge plans incorporating patient and family goals. Document evaluation and plan of care according to departmental guidelines. Document each treatment session according to departmental protocols. Complete and submit family education, discharge, and other required documentation within facility and program guidelines. Adhere to ethical business practices, demonstrating highest standards of integrity and honesty. Embrace diversity and demonstrate ability to work together. Maintain confidentiality of all patient information. Maintain attendance/punctuality in accordance with system policy. Adhere to departmental dress code policy, including wearing ID badge appropriately. Communicate information effectively verbally and in writing with all team members. Attend and participate in staff meetings, required in‑services, process improvement activities, and other meetings as requested. Organize workload to complete responsibilities in an appropriate and timely manner. Utilize materials and equipment safely and cost effectively. Observe infection prevention precautions.
