Community Paramedic

Overview Under supervision of the Medical Director and Manager or designee, performs the role of Community Paramedic, delivering patient care, training, equipment use and maintenance, and research efforts. Responsibilities Provide assessment and care to patients with chronic medical, prenatal, and behavioral problems. Identify patient needs and connect patients with community and medical resources. Follow established community paramedicine procedures and techniques. Support existing health services by providing integrated health services in partnership with other health professionals. Extend access to health services delivery in underserved and general populations, including primary care, public health, disease management, prevention, and wellness. Assist in development of patient care protocols and methods. Participate in training curricula and research. Conduct post hospital release follow‑up care, including monitoring medications, dressing changes, and vital signs. Observe, record, and report patients’ conditions and reactions to medications or treatments. Conduct patient education on diabetes, hypertension, congestive heart failure, chronic obstructive pulmonary disease, fall assessments, injury evaluation, geriatric frailty visits, nutrition, and nail related treatments. Perform home safety evaluations, blood draws, point‑of‑care testing, and ultrasound. Pickup and deliver patient needs such as medications. Perform essential functions of a paramedic as appropriate. Ensure all community paramedic encounters are documented in EMS ePCR and Epic hospital charting. Maintain clear communication with patients’ families, physicians, and other healthcare personnel. Use electronic health records and general office software. Prepare reports and logs, and document deviations from standard operating procedures. Attend required staff and educational meetings and remain current with literature. Assist in writing presentations and papers for publications and may present findings at conferences. Complete patient documentation in a timely fashion to expedite billing. Perform public education, marketing and outreach. Maintain a clean operational environment, retrieve stocks, clean medical equipment. Respond to on‑call emergencies, disasters, and major incidents. Perform data entry and clerical assistance. Assist with Emergency Department communications and dispatch. Qualifications Current Rhode Island EMT‑Paramedic license with a minimum of 1 year field EMS experience. Current Paramedic certification through the National Registry of Emergency Medical Technicians. Current ACLS, BLS, and PALS certifications (instructor status preferred). Valid and current driver’s license with a clean driving record. Bachelor’s degree in Biological Sciences, Nursing or related field preferred. Proficient reading, writing, oral communication in English. Working knowledge of statistics, research methodology, and computer skills. Effective written and spoken communication while interfacing with employees, patients, hospitals, and community. Adherence to Brown University Health policies and procedures, and safety standards set by Rhode Island Hospital and the Rhode Island Department of Health. Maintain clinical skill proficiency within scope of practice including patient assessment, intra‑osseous cannulation, medication dosing, airway management, ECG monitoring, ventilator management, and other essential paramedic functions. Prepared for unrestricted travel by ambulance, car, truck, boat, airplane, or helicopter. Demonstrated knowledge and skills to provide care to patients across human development stages and cultural patterns. Physical Requirements Operate in cramped quarters, all weather conditions, and hazardous materials. Capable of running, climbing, and carrying medical equipment and lifting patients. Stand erect from a squatting position while lifting 175 pounds with another person. Free of motion sickness and physical ailments that impair consciousness or cognition. Perform standing, walking, running, swimming, balancing, bending, twisting, kneeling, crouching, climbing stairs, and CPR for extended periods. Handle fine motor tasks such as intravenous line insertion, use of a scalpel. Have adequate vision and hearing to perform job tasks and communicate. Experience One year experience in emergency medical care. Work Environment Variable environment including in‑hospital and out‑of‑hospital emergency and non‑emergency care, disaster situations, unknown substances, ill and injured patients, training environments, and travel to conferences across the country.
