RN Case Manager / LPN

As a RN Case Manager / LPN, your ability to manage client care with specific knowledge and experience in bedside care, symptom management, crisis intervention and family intervention are feature assets for this high-profile nursing position. ** Traveling to patients is necessary. (Summit / Portage Counties)

Educational Requirements

Currently licensed as a Registered Nurse (RN or LPN) in the state of Ohio and in good standing with the Board of Nursing.

Position Requirements

• A minimum of 2 years of nursing experience, hospice exp a plus 

Job Specific Details:

• Manages a Patient caseload, up to 12 patients.
• Manages, plans, coordinates, and delivers nursing care to patients.
• Supervises and documents STNAs and licensed practical nurse plans of care, and documents supervisory visits.
• Participates in on-call coverage. On Call is 1 day during the week (M-F) and One Sat and One Sun a month.
• Participates in regularly scheduled interdisciplinary team. IDG is every other week.
• Applies the policies and procedures of the agency and the rules and regulations of Federal, and State regulatory agencies, and ACHC.
• Assesses & evaluates Patients level of care, needs and requirements.
• Participates in agency quality assessment performance improvement (QAPI) programs.
• Assesses the Patients response to the Plan of Care and recommends changes as needed.
• Provide support to Patient/family regarding disease process, end of life and ethical issues.
• Willingly accepts directions from Director of Nursing.
