Concord Village Rehabilitation and Wellness is seeking a Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team in Concord, Ohio.

This is an excellent opportunity for an OT who enjoys providing meaningful, hands-on care in a Skilled Nursing and Long-Term Care setting . You'll work alongside a collaborative interdisciplinary team focused on helping residents maximize their independence, function, and quality of life.

What You'll Do

• Evaluate residents and develop individualized occupational therapy plans of care
• Provide skilled OT services focused on ADLs, functional mobility, strength, safety, and independence
• Establish appropriate goals and monitor resident progress
• Complete accurate and timely documentation using Net Health
• Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team
• Provide supervision and clinical direction to COTAs as appropriate
• Educate residents, families, and caregivers regarding treatment plans and recommendations
• Participate in discharge planning and help promote safe transitions
• Maintain compliance with applicable state, federal, facility, and professional standards

Qualifications

• Graduate of an accredited Occupational Therapy program
• Current Ohio Occupational Therapist license or eligibility for Ohio licensure
• SNF/LTC experience preferred
• Strong clinical, communication, and organizational skills
• Familiarity with Net Health and PCC is a plus
• Passion for working with the geriatric population and helping residents maximize their independence
