CASE MANAGER JOB DESCRIPTION
The Program
The Village Project, Inc. works to break cycles of incarceration and poverty by supporting justice-involved individuals on their path to self-sufficiency.
Our Proposition 47 – Journey to Freedom Program, funded by the California Board of State and Community Corrections (BSCC), provides culturally responsive case management for individuals returning from incarceration, as well as prevention support for those at risk of justice involvement.
Rooted in social justice and equity, we help individuals and families rebuild stability and become contributing members of their communities.
Job Description
The Case Manager reports to the Program Director. The Data Analyst may contact Case Managers regarding program compliance and data collection.
Case Managers at The Village Project, Inc. are responsible for participant recruitment, career planning support, job program assistance, assessing clinical needs, and connecting clients to appropriate services, including traditional or Equine Therapy.
Core Responsibilities
· Attend required training related to program policies, procedures, and job skill enhancement.
· Conduct intake assessments using agency tools and verify program eligibility; provide referrals when appropriate.
· Maintain strict confidentiality in accordance with law and agency policy.
· Collect, manage, and securely maintain client files and data on-site.
· Maintain accurate case notes, track client progress, and utilize agency technology for documentation and communication.
· Communicate with justice system representatives (court, probation, law enforcement) and attend court hearings when appropriate.
· Coordinate with community partners, including the Court Emergency Response Team, faith leaders, NAACP, LULAC, and other stakeholders.
· Conduct weekly client progress meetings and attend regular supervisory meetings.
· Lead monthly Court Emergency Response Team meetings and recruit new members.
· Collaborate with the Evaluator using the agency Logic Model to support outcome tracking and reporting.
· Adhere to all agency policies, procedures, workplans, and funder requirements.

· Familiarity with the client population.
· Professionalism, accountability, and openness to supervision and feedback.
Qualifications
· Strong interviewing and assessment skills.
· Experience with data collection and management systems.
· Ability to interface with compliance consultants and support grant deliverables.
· Live Scan clearance required.
· Ability to adhere to funder workplans and assist in sustaining funding efforts.
Education
· Bachelor’s Degree is preferred in Criminal Justice, Social/Human Services, or related field.
· Relevant lived or professional experience may substitute for a degree.
