Speech-Language Pathologist (Per Diem) – Geriatric & Home-Based Rehabilitation OUTREACH Rehabilitation Manhattan, New York About OUTREACH Rehabilitation OUTREACH Rehabilitation is a well-established multidisciplinary rehabilitation practice that has been serving older adults throughout New York and New Jersey for over 20 years. We specialize in delivering Physical Therapy, Occupational Therapy, and Speech-Language Pathology treatment directly within the home and community setting. OUTREACH bridges the gap between traditional outpatient rehabilitation and home health by providing individualized, patient-centered rehabilitation care in patients’ homes, senior residences, assisted living communities, and office settings. Due to continued growth, we are seeking compassionate, motivated, and clinically driven Speech‑Language Pathologists who are passionate about working with geriatric populations and value flexibility, autonomy, collaboration, and meaningful patient care. Position Summary The Speech‑Language Pathologist will evaluate and treat adult and geriatric patients within the community setting, focusing on communication, cognition, swallowing, speech, language, and overall functional independence. Clinicians work closely with patients, caregivers, and physicians to develop individualized plans of care that support safe, functional, and meaningful outcomes. Key Responsibilities Perform comprehensive speech, language, cognitive, voice, and swallowing evaluations Develop and implement individualized treatment plans Provide speech, cognitive, communication, and dysphagia treatment Recommend appropriate compensatory strategies and therapeutic interventions Educate patients, caregivers, and family members on home programs and carryover strategies Collaborate with Physical Therapists, Occupational Therapists, physicians, caregivers, and interdisciplinary rehabilitation team members Complete timely and accurate electronic documentation in compliance with regulatory and company standards Qualifications Master’s Degree in Speech‑Language Pathology from an accredited program Preferred Clinical Experience Dysphagia management and swallowing rehabilitation Cognitive-communication rehabilitation Neurological rehabilitation Voice and speech disorders Geriatric rehabilitation Home-based rehabilitation Minimum 1 year of experience working with older adults preferred Strong clinical reasoning, communication, and interpersonal skills Ability to work independently while remaining collaborative within a team environmentHome care, home health, outpatient geriatrics, or community-based rehabilitation experience preferred Competitive unit-based compensation structure with treatment reimbursement up to $126 per session based on billed units Comprehensive onboarding, mentorship, and clinical support Collaborative and supportive team culture Why Join OUTREACH?
