About Hospice Care Partners

Hospice Care Partners (HCP) is a Medicare-certified hospice agency based in Irving, Texas. We are a hands-on, growing team focused on comfort, dignity, and responsive support for patients and families. We are expanding into South Dallas and building a strong clinical team under our new Director of Nursing.

The Role

As a Hospice RN Case Manager, you will lead the plan of care for a caseload of hospice patients in the South Dallas / Duncanville area, in homes, assisted living communities, and nursing facilities. You will report to our Director of Nursing and work closely with our Medical Director, nurse practitioner, social workers, chaplain, and hospice aides.

What You’ll Do

• Complete admission and comprehensive assessments, and update the plan of care through recertification
• Case manage assigned patients: symptom management, ongoing visits, and timely care plan updates
• Support and educate patients and families, including guidance through the end-of-life process
• Participate in weekly IDT meetings and coordinate care across the team
• Communicate with facility staff and referral partners in your territory
• Document accurately and on time in our EMR (Consolo)
• Participate in on-call coverage [rotating weekends]

What We’re Looking For

• Active Texas or compact multistate RN license in good standing
• Hospice or home health experience preferred; strong med-surg or acute care experience considered
• Sound clinical judgment and comfort working independently in the field
• Strong documentation habits and follow-through
• Dependable, with reliable transportation, a valid driver’s license, and current auto insurance
• Consolo EMR experience is a plus

Our Hiring Process

Phone screen, then an in-person interview at our Irving office that includes a short clinical case-study exercise. Conditional offers are followed by reference checks and onboarding paperwork.
