Details Home health setting serving patients in Union and Somerset County, NJ Caseload includes home health and hospice patients across all age groups Full-time day shift schedule: Monday–Friday, 8:00 AM–4:30 PM (7.5 hours/day, 38 hours/week) Epic EMR used; experience strongly preferred Homecare Homebase software experience required No on-call responsibilities Requirements Active NJ RN license (or compact license) required Current CPR/BLS certification required Current driver's license, reliable transportation, and auto insurance required Minimum 1–2 years of RN clinical experience required Minimum 2 years of home health experience required OASIS experience required Homecare Homebase software experience required Proficiency in wound staging and treatment, IV care, venipuncture, phlebotomy, inhalation therapy, catheter and drain maintenance, and bowel/bladder training programs Graduate of an accredited ADN or Nursing Diploma program required; BSN preferred CHPN certification preferred Two professional references required for consideration Candidates must reside more than 50 miles from the facility to be eligible Additional Information Administer skilled nursing care to home health and hospice patients in their place of residence Develop, implement, document, and evaluate individualized Plans of Care (POC) Supervise and provide clinical direction to HHAs, STNAs, and LVNs Coordinate care with interdisciplinary teams, patients and families, physicians, and referring agencies Educate patients and families on care, medications, diet, and home safety using age- and culture-appropriate teaching methods Attend interdisciplinary team meetings as needed Floating to a nearby sister facility may be required Travelers who have been on assignment for one continuous year must take a 30–60 day break before beginning a new assignment at this facility American Traveler Job ID #P-721266.
