On Call Registered Nurse (RN) - Hospice

Job Description: RN Hospice Case Manager – Per Diem Perform comprehensive nursing evaluations. Assess for signs and symptoms of discomfort including presence of pain and take prompt and appropriate action. Initiate and manage plan of care, provide appropriate nursing follow‑up to all patients, and instruct caregivers in managing patient needs. Coordinate all services provided to patients and families, as well as appropriateness of care, to ensure quality of care and services. Qualifications Bachelor’s degree in Nursing preferred; graduate of an accredited school of nursing. Current RN license in California. Certification in Hospice and Palliative Nursing desired. Current CPR certification. Valid driver’s license and auto insurance. Minimum of one year experience as a professional nurse within the last three years with strong clinical assessment skills. Excellent written and oral communication, sympathetic attitude, customer service/relations, ability to work with dying patients, organizational and diplomatic skills. Supervisory Responsibilities Provide supervision of Home Health Aides, Licensed Vocational Nurses, Licensed Practical Nurses, and Homemakers to ensure the best quality of care for patients. Pay $50 – $58 per hour.
