Role Overview Provides direct patient care in an ambulatory setting, collaborating with providers and the care team to develop and implement individualized care plans that ensure high-quality, cost-effective treatment. Supports clinical staff and assists providers with daily patient management, documentation, and patient education. Key Responsibilities Develops and updates care plans with PCPs and specialists to optimize health outcomes. Assesses patients' physical, psychosocial, and emotional needs using critical nursing skills. Ensures adherence to clinical protocols, documentation standards, and compliance guidelines. Educates patients and families, providing verbal and written instructions, assessing their understanding, and problem-solving as needed. Supports a safety-conscious work environment, maintaining alertness and safety practices. Qualifications & Skills Education: Nursing Diploma/Associate's degree; Bachelor's preferred. Experience: 2-5 years; 5-10 years preferred for advanced roles. Licenses & Certifications: RN license, Basic Life Support (BLS).
