Roles & Responsibilities

• Evaluate residents and create personalized treatment plans to improve their functional independence
• Implement therapeutic interventions under physician orders, ensuring safety and efficacy
• Collaborate with the interdisciplinary team to coordinate care and discharge planning
• Document progress in the electronic medical record with attention to Medicare and other regulations
• Educate residents and caregivers on home exercise programs and assistive devices
• Supervise and mentor Physical Therapist Assistants and new staff

Qualifications

• Active Physical Therapist license in Massachusetts required
• Minimum 3 years of experience in a skilled nursing or long-term care setting required
• BLS certification preferred (but we can help you get it)
• Strong communication and teamwork skills to thrive in a collaborative environment

Education

• Bachelor's degree in Physical Therapy required; Master's or Doctorate preferred

Knowledge & Skills

• Expertise in patient evaluation, treatment planning, and rehabilitation techniques
• Proficiency in Microsoft Office and electronic medical records
• Excellent time management and documentation abilities
• Compassionate, resident-centered approach to care
