As a Licensed Clinical Social Worker, your responsibilities will include:

• Conducting comprehensive assessments of seniors’ psychosocial needs by evaluating emotional, mental, and social factors influencing their well-being.

• Collaborating with clients and their families to formulate individualized care plans that align with their personal goals, ensuring a tailored approach to care.

• Providing counseling services and emotional support to help seniors navigate life transitions such as loss, grief, or health changes.

• Advocating for clients by facilitating access to community resources, ensuring they receive necessary social services and healthcare support.

• Working closely with interdisciplinary teams—comprised of nurses, caregivers, and other healthcare professionals—to coordinate and optimize care delivery.

• Monitoring client conditions through regular visits, offering proactive solutions to address safety issues, living conditions, and overall health.

• Facilitating effective communication among family members involved in care, resolving conflicts, and fostering a supportive environment.

• Educating clients and families about available community resources, support groups, and programs that promote social engagement and improve quality of life.

• Maintaining thorough and accurate documentation of assessments, care plans, progress notes, and any significant interactions with clients.

• Staying current with social work practices and ethical standards, ensuring compliance with laws and regulations governing the profession.

• A Master’s degree in Social Work from an accredited institution.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Georgia.

• Deep understanding of social work principles, aging-related issues, and the breadth of available community resources.

Experience working in a community health clinic or similar environment that serves seniors will be highly valued. Existing knowledge of local resources and networking within the senior services community is also a plus, enabling you to better connect clients with additional support as needed.
