Physical Therapist - Per Diem

We are seeking a skilled Per Diem Orthopedic Home Physical Therapist to evaluate and treat patients recovering from total hip and knee replacements and other orthopedic conditions in a home setting. Key responsibilities: Educate patients on safe mobility, transfers, and ambulation; conduct initial evaluations, develop and adjust individualized treatment plans; perform functional training including ADLs/IADLs; document patient progress following regulatory and organizational guidelines; collaborate with interdisciplinary teams; and ensure safe, effective interventions. Qualifications: Bachelor's or Master's Degree required, Doctorate preferred; licensed in the practicing state; completed CEU requirements; CPR certification; strong communication, clinical decision-making, and computer skills; experience with EMR software is a plus. Skills & Attributes: Compassionate, team-oriented, detail-focused, dedicated, and motivated by patient success and professional growth.
