Compensation : Mileage reimbursed at $0.55/mile, and Payroll processed bi-weekly (26 pay periods).

Shift Type: Full-time, In the field/On the road at 15-30% of the average 40 hour work week.

Summary:

The Hospice Medical Social Worker (MSW) provides compassionate psychosocial support to patients and their families throughout the hospice experience. The MSW assesses emotional, social, financial, and environmental needs and develops individualized plans of care in collaboration with the interdisciplinary hospice team. The MSW serves as an advocate and resource for patients and families while helping them navigate end-of-life care, grief, loss, and available community resources.

Responsibilities:

• Conduct comprehensive psychosocial assessments of hospice patients and families, identifying emotional, social, financial, environmental, and practical needs that may affect quality of life or the plan of care.

• Develop and implement individualized social work interventions, providing counseling, emotional support, crisis intervention, anticipatory grief support, and assistance with end-of-life planning and related concerns.

• Educate patients and families regarding the hospice philosophy, services, dying process, advance care planning, and available resources while supporting patient and family preferences, dignity, and quality of life.

• Connect patients and families with appropriate community resources, financial assistance programs, transportation services, support groups, and other social services; advocate for patients and families as needed.

• Collaborate with physicians, nurses, chaplains, aides, volunteers, and other members of the interdisciplinary team to address psychosocial needs, complex family dynamics, communication challenges, and patient or family crises.

• Provide bereavement support, grief counseling, and referrals to appropriate grief and community resources for patients, families, and caregivers.

• Complete required assessments, care plans, progress notes, and other clinical documentation accurately and within established timeframes while maintaining confidentiality and compliance with applicable regulations and organizational policies.

• Conduct visits in the patient’s home environment as appropriate; maintain professional relationships with patients, families, caregivers, referral sources, and community agencies.

• Participate in interdisciplinary team meetings, quality improvement activities, staff education, and other organizational initiatives as assigned.

Qualifications:

• Active Medical Social Work licensure in the state of Michigan.

• Master of Social Work (MSW) degree.

• 1 year of experience as a licensed social worker in a healthcare setting is required.

• 2+ years in an acute or post-acute care setting is preferred.
