Nurse Practitioner

Nurse PractitionerThe Nurse Practitioner works within the Harm Reduction framework to provide low-barrier, trauma-informed medical care to individuals experiencing homelessness and those disconnected from traditional healthcare systems. This role delivers mobile medical services across TNM's outreach platforms, including Street Medicine, Health Outreach Bus, CTA outreach, shelter-based services, and youth programming, while bridging individuals to ongoing primary care, specialty care, and other community-based services.The purpose of this role is to deliver comprehensive, harm-reduction–oriented medical care in nontraditional settings; reduce barriers to care for unsheltered and marginalized populations; support engagement in substance use treatment, mental health care, and chronic disease management; strengthen referral pathways and partnerships; ensure compliance with regulatory and funding requirements; and contribute to ongoing program development and quality improvement.Role Qualifications:Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited programLicensed Advanced Practice Nurse in Illinois (Family preferred; Adult-Gerontology or other relevant population focus considered)DEA license or eligibility to obtainMinimum 2+ years of clinical experience; primary care or community health preferredExperience working with individuals experiencing homelessness, substance use disorders, serious mental illness, or complex trauma strongly preferredAbility to work evenings and weekendsValid driver's license and good driving recordPosition Responsibilities:Provide low-barrier, harm-reduction-oriented medical care across TNM outreach settings, including Street Medicine, Health Outreach Bus, CTA outreach, shelters, and youth programsConduct focused assessments and treat acute and chronic medical conditions in mobile and outdoor environmentsInitiate and manage pharmacologic treatment, including medications for opioid use disorder and other substance use treatmentsProvide wound care, vaccinations, preventive services, and health education, including participation in public health response efforts when neededFacilitate referrals and warm handoffs to primary care, specialty care, mental health, substance use treatment, and social servicesMaintain accurate and timely documentation in the Electronic Health Record and required reporting systemsCollaborate with interdisciplinary outreach staff to coordinate client care and engagementSupport medical supply management and medication inventory for outreach platformsProvide clinical guidance to medical students, residents, and volunteersParticipate in quality improvement efforts, training, and required agency meetingsTravel to various outreach locations and work in outdoor environments as requiredPerform additional duties as assignedEssential Skills and Experience:Strong commitment to harm reduction and trauma-informed careDemonstrated experience working with individuals with substance use disordersComfort practicing medicine in nontraditional, resource-limited environmentsAbility to make sound clinical decisions independentlyExcellent interpersonal and de-escalation skillsAbility to exercise sound judgment and respond calmly in dynamic and emergency situationsStrong organizational skills and attention to documentation detailFamiliarity with community referral networks and safety-net systemsCultural humility and commitment to health equityCollaborative team mindset with flexibility to adapt to changing schedules, environments, and interdisciplinary outreach needsPreferred, Non-Essential Skills:Spanish language proficiencyExperience in street medicine or mobile healthExperience with MAT induction and maintenanceExperience working in shelters, syringe service programs, or community-based outreachExperience supervising learners or volunteersReporting to this position: No Direct ReportsPhysical Demands, Work Environment, & Reasonable Accommodation:The physical demands and work environment characteristics described below are representative of those that must be met by an employee to successfully perform the essential functions of this position.
